Q: Is the widest part of the opening supposed to be perfectly flush?

Its almost flush, but first I had to rub a little bit of olive oil around the outside of the flange and pushed them in firmly. That was the quickest way to get them placed in.

Q: Will these inserts ever come in a 17 or 19mm like the other pump parts you provide? A 21 is still too large

Freemie offers an OEM insert for their collection cups down as small as 15 mm, although price is considerably more than Maymom.

Q: how often should cushions be replaced?

I personally didn’t replace them unless they tore because I washed them after every use

Yes it does. It's a tight fit but you can make it work.
