Q: can we turn off Output Voltage while Input Voltage still on?

Short answer: No. Long answer: It has to do with the circuit the manufacturer choose, but the lowest voltage output it can be is 3.5V. If you want to turn off the output, then you will need to look for a board with that capability. Or you can add some additional circuitry to the output supply, like a switch, or 5v relay, or an op-amp circuit.

Q: any recommendations for affixing the included heat sink?

There is a tape sticiking under the heat sink, just remove the tape and then stick the heat sink on the converter's surface.

Product arrived falling apart

Opened the package one of the CS 470 35V pots solder didn't hold and was loose in the bag along with the heat sink bar was never attached. re-soldered the pot but haven't tried the device to see if it works or not yet.
