Hello and thank you for asking. The regular size fits people with a chest circumference between 24" to 48", while the X-Large is mostly for people with a chest circumference between 40" and 58".

Q: Does this help with keeping shoulders down? I am not kyphotic nor suffering back issues but my neck and shoulders are a problem.

It primarily pulls the shoulders back, and makes you stand up straighter as a result. It helps you raise your chest in line, so I suppose your shoulders would drop a bit, but its not like it pushes them down. As far as I can tell, the only effect on the lower back is it may get tired if it's used to slumping. Hope this helps.
