Q: Can i add the hook to a d-ring i already have? how do i attach it?

The hooks are fixed to the d-ring, you can't take them off.

Q: Are these made in the USA?

Sorry I'm out of town right now, so I can't check. They are good quality and I recommend though.

Q: Is it safe for them to go through a Washing machine and dryer frequently?

I used them on coin purses and cellphone purses with wrist handles. I wash them every couple weeks. No problem so far.

Mine is in a clear ziplock bag. Hope this helps. It came in a padded envelope in the clear ziplock bag.
