where are the holes for mounting? need holes at an offset above and below center of control knob.

I have the same issue with the offset screw holes, did you ever get an answer? It is offset like this.. ,o' with the o being the valve

what kind of material is it made of

Stamped tin with chrome finish. There are alot of universal replacements but the Moen is the best as it original piece. Other intimidators are plastic & many times they are constructed incorrectly so you don't have hot water stops.

does the T470 come with cartridge and and stop tube

Mine did not have the stop tube, but I was able to order it independently. Not sure what a cartridge is in this application...

Does the trim piece have a gasket on the back or do you need to caulk it?

Even with the "gasket" provided, you'd still want to caulk around the edge.... I used a thin bead of clear 100% silicone.
