It is a little confusing at first because it never actually feels cold on the inside, but it is in fact keeping it cooler than if an insulin vial was sitting on the outside. I love that I NEVER have to think about restoring the coolness. Just drop it in the bag and forget about it. Warning that it is a bit on the heavier side due to the cooling technology so I am glad I got the smaller one.

The phase change material doesn't last through the day during this past heat wave and didn't resolidify overnight in the room. Had to start putting the whole thing in the fridge for the night and most of the day, taking it out a few hours before it is needed. It might be worthwhile to purchase the larger version, though I do admit a heat wave may not be fair but remember when traveling, you won't always have a refrigerator to reset the PCM and it appears to take longer than overnight at room temperatures. You'll need to buy some sort of thermometer with min/max feature so you can tell how hot it really became. I first tried a fridge thermometer but although it advertised "max/min", it was not the expected "min/max" feature. I wound up with a Switchbot Bluetooth thermometer/hygrometer which allowed an app on my phone to graph the temperatures. The fridge magnet had to be force fitted so most fridge magnets would be an awkward fit, but the Switchbot was the right size.
