Q: where are these filter made - China or USA?

These are made in CHINA. The description made me believe it was made in the USA. Right on the package it states made in a Chinese factory. Almost the entire packaging is in Chinese.

Q: Which side of the filter faces towards the face?

No distinction is made between positive and negative. It works on both sides. No need to point

Q: How often should you changeout your filter?

I tend to switch the position front to back once and then remove the filter and toss it. Total use maybe three times depending how long I used it. Supposedly it can go in the wash with the mask. I haven’t tried that.
