Q: can this be used under a bathing suit?

Depends on what you mean by 'used'. It could certainly be worn under a bathing suit, but I wouldn't expect it to completely seal out water. If someone was going to swim and be completely submerged in water, I would expect that underwear or briefs worn underneath the vinyl pants would become completely soaked. However, if the suit was worn just while sitting at the beach or boat riding, there shouldn't be a problem.

Q: Do these run true to size?

I ordered Large and when I opened it I thought it was awful big, but I put these on an Medium size elderly man and it was snug. I would say true to size if the person is mobile. For me the gentleman is in a wheel chair so putting on and off would be easier to get a larger size for easy on and off. Hope this helps.

Q: are the Waterproof Vinyl soft to the touch and fit without giving a rash around the leg band?

Leg bands are tight to stop leakage. The vinyl itself is soft and pliable. Leg chaffing could occur and that could lead to rash with short term exposer to urine/bowel incontinence. Best guess on my part.

Q: Are these pool diapers or covers for diapers?

They are Clear Vinyl Plastic Plastic Pants for protection OVER a cloth or disposable diaper. It is NOT a Swim Diaper or Cover
