Can these be used outdoors?

The risers are hard plastic and should hold up under most weather conditions, but I can't say for how long. They could be subject to shattering in extreme cold weather. I actually could not use this product as intended. The top of product was too small and tge two vertical edges didn't allow it to sit squarely under the beds footing. I cut strips of pine, glued them then stacked the to the height needed. Worked great. For another bed I purchased round pcs of wood from a craft store and did the same. Work great.

Will this work for a bed with just a box spring?

You would probably need more than just the standard 4 pieces that come in the package. You would need support for the center of the box springs as well as the corners.I have them on my couch now and they are very good.

Can these be painted to match the legs of my sofa?

I'm not sure. But if I had to guess I would say yes
