Q: is there an upgrade available for this with better grip?

I am not certain of how well it actually works but Philips Respironics did redesign the headgear with 2 "arms" that changes where it sits on your head. You can search "dreamwear headgear with arms" to find it.

Q: Is the inside of the strap (the side touching your head) exposed foam or fabric?

The side against the head is definately a rubbery foam type material. The side away from the head is a fleece material.
