Q: I wear a 6-7/8” cap.. will it be loose?

Same size for me. The scarf is very elastic; fits me comfortably. Not loose in the sense of sliding off. With wear and washing it may shrink a bit (tighten up). So far it is excellent. Could be the "large size" as advertised fits medium to large given the elasticity.

Q: Is the print on the front only or front and back?

It‘s front and back，so even on the back，you will look pretty cool，Two-sided print can be worn for a longer period of time
