The offers is only 10 in a pack. my wood door need 12 rollers. like them so far. where i can buy additional 2 rollers?

This is why so many people are ordering 10 packs and getting 8 rollers - people are buying an additional box, taking 2 and returning the box as 'new'. The next person buys a 'new' item and is shorted. Really dishonest and giving the product and seller a bad name. So please, order the 12 and dont short a box and return it as new.

The package shows plastic caps that snap on to each roller. But none were included in the box. Were they suppose to be there or is that separate?

As Bill Boles and WESSOL said, the caps are already on the rollers. The picture just shows how a cap is placed on the roller. I did have to press down on the caps, however, because some weren’t fully seated. Whoever assembled the caps to the rollers at the manufacturer didn’t do that good a job!

What is the diameter of the shaft?

.430 diameter x 4 1/4" long to shoulder (nearest side) of wheel.

How to order Pack of 12 rollers (prices per roller for a total of $28.74)?

These rollers are already packaged in boxes of 10 in Amazon's fulfillment centers, so unfortunately we are not able to sell you a 12-pack at the moment. However we will be releasing a 12-pack in a few weeks.
