I’m not sure. But I wear a 7 & 7/8” western hat and this hard hat fits well

If your talking about the strap to your head that keeps the helmet off your head. Yes it’s comfortable and tightens with a knob on the back

Q: The insert comes out when I take it off how can this be fixed

Not sure i understand the question. If your saying the suspension system comes our when you remove the hat, you don't have if in correctly. make sure the metal clips snap onto the posts, it may take some good pressure to get them to snap in.
