The shoulder straps adjusters do slip a little if you have a heavy load with oxygen tank though.

This bag has a great amount of storage, it holds almost everything I need as a paramedic. The bag is very comfortable on the back as well. It's build very well, it appears like it will last a long time. The foam is removable, and more importantly replaceable if necessary.
