Yes. It comes with two. Yo can adjust any way your most comfortable. I highly recommend them. Since using I wake up with no stiff neck. Love them

Is a separate cotton pillow case made for this shape pillow?

The pillow case has a zipper. You can take off & wash it. There is another pillow case underneath the zipper one. No zipper on that one. I use my pillow when I sit in a rocker recliner everyday. I put it under my neck. Sometimes I use it when I sleep. I am glad I bought the pillow. It is firm enough to support my neck.
