Q: How many DVD it holds

I found that when I filled the book up with 4 DVD's in each page I could only get 13 pages in there without the book bulging out and potentially damaging the spine. I bought the extra pages but I think the original 12 pages fills the book fairly well. So 48 DVD's with the original pages but no more that 52 with an extra page.

Q: How many DVDs do each of the 3 binders hold? Description says 64, some owners say 12 inserts ea & the 6 book set says 288. Anyone?

1 binder has 12 inserts. Each insert can hold 4 disc. Which means, each binder can hold 48 disc. The maximum pages you can insert in 1 binder is 16 (total of 64 disc). Additional pages are sold separately.

Q: are the pages 'lined' ? or is there plastic-on-plastic (data side of dvd)?

Yes there is a paper or fabric lining. I have found with children or the inattentive that you have to hold it upright. If you don't then all the DVDs will fall out.

Q: are the sleeves large enough to allow you to keep the cd cover art as well

Yes for cd's. No for Dvd's. You would have to trim it. Each sleeve measures 5" x 5". But there is on the left side of the sleeve where you can trim the cover of the spine and put it there. That measures 1" in width and 5" in length.
