Q: Does anyone know if we're supposed to keep the client face down during the whole massage? Or do we flip them over without removing the cushion?

No I don't think they should be faced down the whole time nor do I think it would be comfortable to flip over on top of it. What I do is hand her a pillowcase and she covers her chest with one arm. She uses her other arm to rise up on all fours, or threes (knees and one arm), then I pull the cushion out and she flips. You may want to get a step to use while she is on the cushion (like used for step class, they are at Walmart) because it makes them much taller.
