Can this be worn with shoes

No, not at all. I found these completely useless - they just slid off the toe, and are so bulky they hurt. I'm looking into what they call a hammer toe crutch, which looks much more effective and can be worn with shoes. Haven't got it yet, but I hope it will be more effective.

Where can I buy and extra long aluminum bar?

Toe splints have two long bars in each package, it should be enough for spare.
