Does this card have "fan stop" or do the fans run at some minimum level (like 1500 rpm) all the time, even at idle?

I have it and I do believe that the fans are always running from the moment I power it on, however, they are inaudible at idle

Will this run off of motherboard power or does it need power directly from the PSU?

The motherboard only provides 75W and its a 115W card. It takes an 8 pin pcie power connector however, if your psu only has a 6 pin you can use an adapter to 8 pin, that's what I did.
